Silver Ridge Spa - Guest Forms

At Silver Ridge Spa your Health and Wellness is very important to us. Upon your first visit to Silver Ridge Spa, we will ask you to complete one or more forms that will detail all the information we require before we are able to perform services on you; this includes Personal Information such as current and past health conditions as well as contact information. This will insure that our staff R.M.T.'s and Estheticians are aware of any current/prior conditions and contra-indications, so we may provide you the proper treatment

NOTE - if you are under 18, the forms must be completed and signed by your parent or legal guardian.

Please download the forms below and complete them before you come to our Spa. This will help improve your experience by speeding up your initial processing time upon your first visit.  Simply give them to our Concierge when you come in.

ALL new guests that visit Silver Ridge Spa for the first time are required to complete this form. Our Staff update the second sheet after each visit.
